  • 跌倒會造成長者生理與心理的傷害,先前研究較多關注長者跌倒危險因子與傷害。對於服務提供者執行預防跌倒的研究主要以醫院專業人員為主,較少針對日照中心之服務提供者進行探討。本研究探討日照中心服務提供者處理長者跌倒與執行防跌措施的經驗。
    本研究採用質性研究法。參與者為總工作經驗至少三年且訪談時仍為日照中心在職者,包括業務負責人、照顧服務員、護理師等。運用半結構式與一對一訪談進行資料收集,包含日照中心長者跌倒之高風險因子、跌倒評估經驗、長者跌倒處置流程、預防跌倒措施內容、防跌科技之使用經驗與看法、導入防跌科技的考量。使用主題分析法分析資料,並以三角測定法、同儕檢核確保研究品質。
    研究共招募21位參與者,以臺灣南部日照中心服務提供者為主,年齡範圍25-54歲,7位男性。教育背景以社工相關科系6位為最多,職務以照顧服務員11位最多,業務負責人8位次之,且過半服務提供者有使用防跌科技的經驗。研究結果發現四個主題。第一、跌倒事件的多元成因與影響。長者跌倒原因包含疾病與功能退化、個人行為及環境因素,並非單一因素造成;負面影響有照顧需求發生轉變、跌倒恐懼造成長者活動參與下降等。另外,部分長者會願意開始使用輔具、尋求協助等。第二、防跌措施的執行挑戰。服務提供者面臨人力不足、經驗與技巧缺乏、環境限制,與家屬的壓力,會造成措施執行難度增加。他們亦需要在長者復能與跌倒風險之間取得平衡。第三,預防跌倒的策略。由多元背景的服務提供者合作,使用多重元素的跌倒風險評估,包含疾病、藥物史和長者活動能力等。建置完善的跌倒事件處理流程,在環境設計與照護流程進行調整,並以定期的會議與在職教育改善防跌措施。第四,科技防跌導入的考量。服務提供者認為科技具有輔助功能,例如:早期偵測長者異常、多元整合功能,能減輕照顧負荷。但導入防跌科技需要考量成本效益、可行性、使用者接受度、裝置異常的責任歸屬等因素,也需要科技專業人員協助導入。
    統整服務提供者對於處理長者跌倒與執行防跌措施的經驗顯示,長者跌倒受到多重因素交互影響。服務提供者在執行防跌措施時面臨人力、環境、家屬、以及照顧目標與流程的挑戰。服務提供者依據長者特性與空間限制擬定多項策略,如跨領域合作進行在職教育,強化跌倒風險評估機制、全面檢視中心空間,改善視線死角與高風險區域之環境安全、定期會議改善標準化照護流程、及調整人力配置以提升照護效率。科技亦可作為防跌輔助工具以減輕照顧負荷,但須降低錯誤警報以及滿足成本效益與方便使用之特性。重視第一線服務提供者的經驗與需求,作為科技設計與政策支持的參考,以改善長者的安全和生活品質。

    Falls can cause serious physical and psychological consequences for older adults. This study explored contextual analysis and coping strategy for fall incidents among care providers in day care centers. Semi-structured interviews were conducted with 21 care providers, all with at least three years of experience providing direct care to older adults. Four themes were identified: (1) The multiple causes and effects of falls: Falls resulted from a combination of physical, behavioral, and environmental factors, and their consequences were both positive and negative. (2) Challenges in fall prevention: Barriers included limited staffing, inadequate training, environmental constraints, and the difficulty of balancing safety with older adults’ rehabilitation needs. (3) Fall prevention strategies: Care providers had established structured post-fall procedures and contributed diverse professional insights into environmental design and care planning. On-the-job education helped enhance their understanding of risk assessments and reinforced prevention practices. (4) Considerations for implementing fall prevention technology: Technology was perceived as a supportive tool to assist rather than replace care providers. Participants recognized its potential to reduce caregiving burdens and improve fall prevention, but they also concerned the feasibility and accuracy. Although current strategies primarily rely on human resources, the integration of technology may offer a practical path forward in enhancing safety in day care centers.
