  • 研究背景
    全球的糖尿病人口數已超過4.5億人,糖尿病自我管理已成為一個重要公衛議題。目前,有越來越多以科技提供健康服務與疾病管理的工具,然而現有疾病管理科技開發缺乏醫療專業人員在設計過程中的角色,同時欠缺這些科技醫療與現實生活、環境間的連結及測試。除此之外,近年來照護相關的機器人開發缺少實證、實務與專業的共同建構。另外科技結合社區的醫療資源,例如:可近性高的社區藥局,了解以科技輔助來填補醫療機構與個人照護上的落差,也尚未有明確證據。。
    研究目的與方法
    目的一 設計並開發以符合中高齡糖尿病患者需求及社區藥事照護觀點之糖尿病照護機器人原型
    目的二 評估中高齡糖尿病病人使用原型機器人後的滿意度、糖尿病知識、自我管理和態度的改變及對於機器人之整體評價與建議
    目的三 探究社區藥局藥師對於原型機器人於社區糖尿病照護應用的整體與評價
    本研究擬建置糖尿病照護機器人原型;並採用質量性混合研究方法(mixed-methods research)進行剖析。
    (1)設計系統:以符合中高齡科技與糖尿病疾病管理之研究理論為核心概念,並佐以社區藥局藥師、糖尿病衛教中心的實務經驗。結合工程師、醫師、老年學領域專家與科技領域專家進行約每周一次的原型機器人開發會議,經由團隊針對機器人的外型、內容、功能與介面進行方向探討與設計。
    (2)量性評量:收案條件為45歲以上的第二型糖尿病患者。以糖尿病知識問卷、糖尿病自我效能量表及社區藥局可行性量表為前測問卷,並使糖尿病人與機器人互動後進行後測,後測問卷另增添滿意度調查問卷。
    (3)質性評量:採半結構式質性個別訪談糖尿病病人及社區藥局藥師。主題架構為對於機器人的建議與想法、對於疾病照護或社區照護的需求及對未來健康照護科技的期待。
    研究結果
    糖尿病人參與者共計30位,平均年齡69.2±9.69;參與的社區藥局藥師10位,平均年齡49.6± 14.06。糖尿病病人在與機器人互動後,在糖尿病知識(p<.001)、機器人的應用可行性(p=.004)以及認為機器人有機會協助提升自我照顧能力(p=.047)在後測分數上皆比前測有顯著提升。此外,糖尿病知識、機器人應用可行性、機器人提升自我照顧能力的看法與滿意度上,在前後測的結果中,並無因為性別、年齡與糖尿病病史的不同而呈現顯著的差異。但是在教育程度較高者,於糖尿病知識的後測分數顯著高於教育程度較低者(p=.006);另外,於滿意度量表中的“與社區藥局機器人互動可以促進我與社區藥局藥師的交流”此項,有行動應用程式使用經驗的滿意程度顯著高於缺乏經驗者(p=.033)。在根據質性訪談結果,糖尿病病人呈現五大主題:主題一滿足中高齡自我導向學習的機會;主題二降低戒心,營造舒適互動;主題三生動與豐富增進互動機會;主題四機器人應用並非無往不利與主題五功能應用各有所好。藥局藥師訪談歸納出三個主題: 主題一科技的開發皆須從病人的角度出發;主題二締造新服務與主題三機器人的應用需呼應現實。
    結論
    本研究支持經由糖尿病七大照護理論以及跨專業團隊會議所開發設計的原型機器人架構,儘管因為機器人軟硬體設備的限制、網路的穩定度,會因而限縮機器人的開發功能,然而初步所建立的模板與開發理念是被糖尿病病人與社區藥局藥師所接受的,甚至因而能更深入了解社區糖尿病照護的需求。此外,也發現機器人在社區衛教應用上,於糖尿病知識加強部份確實有 其可發展的契機。建議未來建立長期的介入與評估研究,才得以了解糖尿病管理與科技連結的長期應用成效,同時需確立醫療與科技連結的完整應用模式,以利科技在健康照護上得以永續。

    SUMMARY
    The research aim of our study is to design and develop a prototype diabetes care robot that meets the needs of middle-aged and older patients with diabetes and the perspective of community care. And to assess the satisfaction, diabetes knowledge, self-management and attitude changes of middle-aged and older patients with diabetes after using prototype robot, and recommendations for robot. Finally, we explore the overall evaluation of community pharmacists on the use of prototype robots in community diabetes care. There were 30 diabetes patients and 10 community pharmacists participated in the study. The results showed a significant improvement in diabetes knowledge (p<.001), self-efficacy(p=.047) and feasibility of the robot(p=.004) after interacting with the robot. Five themes emerged from interviewing diabetes patients: Theme 1 meets the needs of self-directed learning for the elderly; Theme 2 reduces alertness and creates comfortable interaction; Theme 3 enhance vividness and richness interaction opportunities; Theme 4 robot are not without disadvantages and Theme 5 every man has his taste. Three themes emerged from interviewing pharmacists: Theme 1 technology must meet the real needs of the patient; theme 2 creates new services and theme 3 function and reality must be balanced. The findings address that whether it is a community pharmacist or a diabetes patient, the use of robots is acceptable, and is considered feasible for robots to promote their health care in the future.
