  • 研究背景:適當展現同理心能促進良好的護病關係。加護病房護理人員在臨床工作中,因病人疾病嚴重程度高,臨床工作技術複雜,容易增加護理人員工作壓力導致無法展現適當的同理心,而影響護病溝通。而病人及家屬因面對加護病房陌生環境的焦慮,當內心需求未被理解及協助時,會導致護病關係信任感的降低,影響護理服務滿意度。
    研究目的:了解加護病房護理人員同理心的現況及探討加護病房護理人員同理心展現之影響因素。
    研究方法:本研究採橫斷式相關性研究,收集資料以結構式問卷調查,包含基本人口學問卷資料及傑佛遜同理心量表-醫護人員版。以方便取樣於南部某區域教學醫院的加護病房護理人員為研究對象,共計發放168份問卷,問卷回收正確率為95.8%。資料分析以統計軟體IBM SPSS 24.0分析。描述性統計,含次數分配、百分比、平均差及標準差等分析,描述各個研究變項;推論性統計則以獨立樣本 t 檢定、單因子變異數分析、皮爾森相關分析及多元逐步迴歸分析,探討影響護理人員同理心的相關因素。
    研究結果:研究對象共納入161位,以年齡越大、女性、已婚、總工作年資越久、護理進階職級N3以上、輪班以固定白班、有接受相關同理心教育訓練及閱讀相關人文素的書籍者其同理心有相關性,在統計學上有顯著意義(p < .05)。經多元逐步迴歸分析後以進階職級及閱讀相關人文素的書籍習慣( p < .005)為同理心之預測因素。
    結論/實務應用:加護病房護理人員同理心現況,為中上程度。建議臨床鼓勵護理人員參與護理人員參與進階的意願,主動學習以提升臨床專業的照護能力。在職教育舉辦可以運用相關情境體驗模擬的同理心訓練課程,以強化臨床照護能力,促進臨床護理人員同理心的展現;在臨床單位養成每月一書閱讀相關人文素養的書籍的閱讀習慣,以促進護理人員同理心的展現,建立良好的護病關係。

    Adequate empathy enabled the promotion of excellent nurse-patient relationships. As higher patient disease severity in the intensive care unit (ICU) and clinical work techniques complexity, the nursing staffs are unable to express suitable empathy due to work pressure. A cross-sectional study was conducted. A convenience sample of 168 ICU nurses was recruited from Southern reginal teaching hospital with 95.8% of response rate. Questionnaires were used to examine the following variables: demographic, work related experienced surveys, and the Jefferson scale of Empathy (JSE). Data analysis involved descriptive statistic to describe demographic, work related experienced survey and empathy status. Factors affecting the nursing staff’s empathy were discussed through inferential statistics. Multiple stepwise regression analysis was used for identifying the predictors of empathy expression of ICU nurses. Total of 161 subjects were included. The overall average score of empathy was 113.5 ± 13.71 points. Statistical significance (p < .05) was found between empathy and older age, female, married, higher total work seniority, nursing Clinical Ladder System greater than N3, fixed morning shift, received relevant empathy educational training and reading humanistic quality book. The Multiple Stepwise Regression Analysis revealed that the Clinical Ladder System and reading humanity literature were regarded as predictive factors (p < .005). The current level of ICU nursing staff empathy was above average. It is recommended clinically to encourage the nursing staff participation and willingness in the Clinical Ladder System involvement. In-service training can utilize simulation of similar scenario training for empathy to strengthen clinical care ability and promote the nursing staff’s clinical exhibition of empathy; Established one humanistic quality book reading each month in the clinical unit can enhance the nursing staff’s performance in empathy and create excellent nurse-patient relationships.
