背景:母乳是提供早產兒免疫力之重要來源,且可促進早產兒神經發育、降低併發症、減少死亡率、縮短住院天數、促進親子關係。過去研究多探討不同護理指導方式對於早產兒母乳哺育率影響,尚未結合行動應用程式及其成效的相關文獻,本研究目的為探討「母乳哺給讚」行動應用程式對早產兒母親產後母乳哺餵的成效。
方法:實驗性研究設計,收案對象為產下妊娠週數≤37週的早產兒母親,其早產兒在嬰兒室或嬰兒病房接受治療。產後生理狀況穩定有哺乳意願,具上網功能的智慧型手機,及中文聽說讀寫能力者。根據研究目的依有無哺乳經驗作分為兩組,進行區間隨機分配,實驗組接受「母乳哺給讚」APP服務,控制組使用紙本「母乳自我紀錄表」,在產後14天內收集兩組每天擠乳次數、母乳收集量和親餵次數,另針對實驗組測量APP使用滿意度。在產後30天讓產婦接受母乳哺餵社會支持量表、母乳哺餵自我效能簡式量表測量,統計早產兒出生一個月內住院天數資料,並由電子病歷或電訪方式追蹤產後一個月純母乳哺育率。
結果:完成研究的早產兒母親人數共60位,實驗組及控制組各30人。研究結果發現產後14天實驗組總擠乳次數(69.4±14.1 v.s. 55.7±22.7, t=2.79, p= .007)、總母乳收集量(ml)(3944±2623 v.s. 2366±1918, u=283, p= .014)與產後一個月純母乳哺育率(43.3% v.s. 20.0%, χ2=10.3, p= .005)均較控制組高;兩組母乳哺餵自我效能量表得分無顯著差異。
結論/建議:本研究結果支持「母乳哺給讚」APP能有效提升早產兒母親產後14天擠乳次數、產後14天母乳收集量及產後一個月純母乳哺育率,但對母乳哺餵自我效能無影響。建議臨床在產前門診或入院待產時安裝「母乳哺給讚」APP,以幫助泌乳機制建立,提升母乳哺餵成效。
The low breastfeeding rate at one month postpartum for preterm infants is an important and widespread phenomenon. There will be many difficulties in the postpartum period. This study is a randomized control trial to understand ‘Best Feed’APP in numbers of milking times at 14 days postpartum, breast milk collection at 14 days and difference in breastfeeding rate at one month postpartum. But there is no significant difference in Breastfeeding Self-Efficacy Scale-Short Form (BSES-SF). The‘Best Feed’APP can be used at early postpartum use by mothers of preterm infants.
