  •   本研究目的為探討癌症病童主要照顧者之疲倦嚴重程度、焦慮、睡眠品質、細胞健康與疲倦之相關因素,如:照顧者的年齡、病童的年齡及身體症狀。本研究為橫斷式、相關性之研究設計,對象為台灣南部二所醫學中心之癌症兒童主要照顧者,採方便取樣,以結構式問卷收集資料,所使用之研究工具有「個人疲勞強度問卷」用以測量照顧者疲倦的嚴重程度、「情境焦慮量表」測量照顧者之焦慮程度、「體組織分析儀」測量照顧者的細胞健康(相角值)、「匹茲堡睡眠品質指數問卷」測量照顧者的睡眠品質及「修正症狀評估量表」測量病童之身體症狀,並以自擬的結構式問卷取得病童及照顧者的基本資料。本研究共有40位癌症病童之主要照顧者參與,87.5%為女性,其平均年齡37.4±7.7歲。主要照顧者之主觀疲倦經驗之標準化分數為52.9,焦慮程度之標準化分數為62.6,有80.0%的照顧者睡眠品質不良,17.9%的照顧者之相角值低於正常範圍;照顧者之主觀疲倦經驗與其焦慮程度有統計學上顯著正相關(r = .660, p = .000);照顧者之主觀疲倦經驗與其睡眠品質有統計學上顯著正相關(r = .453, p = .003),然而照顧者之主觀疲倦經驗與其年齡、細胞健康、病童之年齡及病童之身體症狀皆無統計學上顯著相關(r分別為 -.154、-.085、.083、-.036,p > .05)。照顧者之焦慮程度可預測其疲倦嚴重程度,解釋量達43.5%。因此,評估主要照顧者焦慮程度宜納入常規的照顧流程中,經由正確的評估與適當的介入措施,以減輕主要照顧者的焦慮程度,並進而降低其疲倦與改善睡眠品質。

      The purpose of the cross-sectional and correlational study was to identify the relationships among fatigue, anxiety, quality of sleep, cellular health, and fatigue-related factors in caregivers of children with cancer at two university hospitals of southern Taiwan. Checklist Individual Strength (Chinese version), State Anxiety Inventory, Impedance-450, Pittsburgh Sleep Quality Index, and a structured questionnaire were used to assess severity of fatigue, anxiety, cellular health (phase angle), and quality of sleep of caregivers, respectively. Adapted Symptom Assessment Scale was used to assess physical symptoms of children. Forty caregivers of children with cancer were recruited using a convenient sample from November 17, 2003 to January 17, 2004. They were 35 (87.5%) female. Average age was 37.4±7.7 years old. The standardized score of subjective fatigue experience and anxiety of caregivers were 52.9 and 62.6, respectively. Less than 20.0% of caregivers were below normal range in phase angle, and 80.0% were poor quality of sleep. The scores of fatigue were significantly correlated with the scores of anxiety of caregivers (r = .660, p = .000). This finding indicated that the more anxiety and the more fatigue of caregivers whose children with cancer. The more fatigue of caregivers was, the poor quality of sleep (r = .453, p = .003). However, there were not significantly correlated with subjective fatigue experience of caregivers and their age, cellular health, children’s age, and children’s symptom distress(r = -.154, -.085, .083, -.036, all p > .05). Anxiety was account for 43.5% of fatigue of caregivers of children with cancer. Therefore, health providers need to assess the anxiety of caregiver, and provide interventions to reduce the level of anxiety, improve the quality of sleep, and reduce severity of fatigue of caregivers.
