在這個越來越重視個人權益的時代, 病歷當屬於個人的觀念已被大眾所接受。個人更應該能掌握自己的健康記錄，不僅能了解自我的健康狀態，看病時更能提供醫師完整的資訊。雲端運算是近年來 IT產業的重要發展方向之一。它是
指將運算、儲存、硬體、軟體、 網路等IT資源，透過虛擬化的資源利用最佳化，以及可量化計費的服務型態，經由網路分送，提供使用者隨時隨地可透過網路存取的服務平台。
本論文將個人健康記錄與雲端運算系統做結合，實作出一套雲端系統上的個人健康記錄管理系統，並提供以國際標準 CDA R2 (Clinical Document Architecture Release 2)格式輸出資料，期望能與國際醫療接軌，讓個人健康記錄能更廣泛的被運用。實作部分包含一基本通用表單及衛生署公布之108表單中之檢查報告、檢驗報告及出院病歷摘要。提供使用者在網路連通的狀況下，可隨時隨地能存取個人健康記錄。

The concept that medical records should belong to the individuals has been accepted by hospitals. It would be beneficial for a person to be able to keep one’s electronic self-health history and to show intact information to doctors. In addition, it would be better for each individual to be able to access one’s own health records via internet. Recently, cloud computing is the emerging technique in IT industry. It provides quantified charge of IT services over the internet while sharing the resources of computing operations, storage, hardware, software, and network. Users are able to access to the cloud service whenever they connect to the internet.
The thesis proposes an approach to combine personal health records and cloud computing, and support the records in international standard format, CDA R2. We hope that it helps the interchanging with international medical treatments and the personal health information can be used more extensively. Current prototype includes general report, comprehensive history and physical note, study report and discharge summary. It allows users to access their personal health records over the internet and provide their health records to doctors.
