一、背景與重要性:醫療人員的醫病溝通形式,除了面對面,電話是最常被使用的通訊設備,COVID-19疫情期間更突顯電話溝通的需求。然而,缺乏溝通訓練和評核標準,使醫療人員在醫病電話溝通時,缺乏信心與應對能力,影響溝通成效。因此,本研究參考近年醫學教育的概念,運用可信賴專業活動(entrustable professional activity, EPA)與里程碑計畫,發展精進醫療人員醫病電話溝通能力的養成與評核策略。
二、研究目的:發展醫病電話溝通可信賴專業活動以及里程碑計畫,確認內容的適切性與重要性,並評值EPA品質。
三、研究方法:本研究第一階段採修正型德爾菲法,發展醫病電話溝通EPA與里程碑計畫。研究者透過廣泛性文獻分析與訪談專家經驗,形成醫病電話溝通的任務描述、核心能力、內涵面向、評核工具與里程碑計畫初稿。採立意取樣,邀請具有醫病電話溝通經驗之專家組成小組,進行共識程序,使用Likert scales 5分量表,針對初稿內容的同意程度以及重要性進行評分,保留眾數5分、超過75%專家評分4分以上、四分位差小於0.6的項目,並參考專家意見作修訂,直到皆達共識門檻。第二階段採立意取樣,邀請EPA專家,使用評估工具Queen’s EPA Quality Rubric(EQual),評值EPA品質,標準切點為平均分數4.07分,表示內容符合EPA架構與良好品質。資料分析方式為描述性統計,呈現專家的共識結果。
四、研究結果:研究第一階段透過文獻分析與訪談8位臨床實務者,彙整出醫病電話溝通的四大面向:溝通開場、資訊傳達、傾聽與同理回應、總結歸納結論,計劃下一步。延伸17項次面向、24項內涵,隨後邀請15位具備醫病電話溝通經驗之專家,進行兩回合專家共識程序,第一回合「同意程度」平均分數3.93~4.79分、評分4分以上專家占比60%~100%、眾數5分、四分位差0~1;「重要性」平均分數4.33 ~ 4.93分、評分4分以上專家占比80%以上、眾數5分、四分位差0~0.5,未達共識標準之題項,依據專家建議進行內容調整。隨後進行第二回合專家共識程序,「同意程度」平均分數4.64~5分;「重要性」平均分數4.71~5分,其餘皆達共識標準。此外,里程碑計畫內容「同意程度」平均分數4.71~5分;「重要性」平均分數4.86~5分,其餘皆達共識標準。研究第二階段,邀請5位EPA專家運用評估工具EQual,評核內容品質,總平均分數4.34分,達標準切點分數4.07分,確認EPA具有標準品質。
五、結論:本研究發展醫病電話溝通EPA和里程碑計畫,內容包括任務描述、核心能力、內涵面向、以及評核工具等,並確立內容具備高度適切性、重要性與品質,未來可運用於建構醫病電話溝通的訓練計畫以及能力評核指標,幫助醫療人員電話溝通能力的養成。
In clinical practice, the medical telephone communication between healthcare staffs and patients is lack of training courses and evaluation standards. By a development of the Entrustable Professional Activity (EPA) and milestone plans based on the competency, this study aimed to enhance healthcare staffs' competencies in medical telephone communication with patients. Aplying the modified Delphi method, We first conducted extensive literature review and interviewed 8 experts who had experience of telephone communication with patients, then there were 15 experts entered two round of experts consensus to develop the EPA competencies, required knowledge, skills, attitudes, milestones and an assessment tool of the medical telephone communication. In the end, there were 86% experts select score 4 and 5; a scoring mode of 5; quartile deviation less than 0.6 and the overall response rate of questionnaire was 100%, which means all experts reached consensus. Second, we invite 5 EPA experts by purposive sampling to evaluate the EPA quality with the tool Queen's EPA Quality Rubric (EQual). The evaluation result of the EPA was 4.34 points, which reached the cutoff point 4.07. Overall, The EPA and milestone plans can be used as a reference for communication training and entrustment decision to enhance competencies of healthcare staffs medical telephone communication.
