目的：哀傷是人類面對失落所產生正常且自然的動態情緒反應過程，但部分喪親者可能持續被強烈痛苦的情緒衝擊轉而發展成複雜性哀傷，宜及早發現需求並進一步轉介專業協助。本文旨在以系統性文獻回顧探究哀傷介入措施對降低複雜性哀傷之成效，透過統合分析發現哀傷介入措施的型態、介入時間長短、介入後評值時間、治療者背景及訓練細節與降低複雜性哀傷之成效的關係，再結合統合分析結果形成臨床建議。
方法：本研究於Embase、Medline-Ovid、CINAHL-EBSCOhost、Cochrane library、Psychology & Behavioral Sciences Collection資料庫以家屬、喪親、哀傷諮商、延長性哀傷障礙、複雜性哀傷、介入措施等關鍵字搜尋2022年8月前發表之文獻，文章納入條件為：(1)針對喪親後家屬執行哀傷介入措施、(2)符合複雜性哀傷之定義、且喪親時間大於六個月以上、(3) 以複雜性哀傷測量工具做為結果指標、(4)大於18歲之成人喪親家屬、(5)研究設計為RCT。排除條件：(1)研究對象非喪親家屬，寵物喪生等予以排除。
本研究將依據文獻搜尋策略進行文獻篩選，以Risk of Bias tools for Randomized trials (RoB 2.0)進行文獻評讀，建立登錄矩陣（coding matrix），矩陣內容含：受訪對象、哀傷介入內容、研究結果及證據等級評讀，確認哀傷介入措施對降低喪親家屬延長性哀傷障礙成效後，進一步剖析哀傷介入措施對象之招募與條件、哀傷介入措施、人員資格與訓練、評值工具與追蹤時間等，將資料登入於Review Manager 5.4系統中，並依此進行統合分析，分析哀傷介入措施的型態、介入措施期程長短等細節與降低複雜性哀傷之成效的關係，以森林圖呈現，並形成臨床應用建議，期能透過專業協助複雜性哀傷喪親家屬順利調適哀傷歷程。
結果：經過系統性搜尋及篩選後，共納入10篇研究進行統合分析討論，研究整體效果量為-1.45（95%CI為-2.11, -0.79；Z=4.3；p<0.00001），顯示哀傷介入措施有助於降低喪親家屬的複雜性哀傷程度。次群組分析中發現，「團體」型態及「8周以上」之哀傷介入在統合分析中不具有顯著效果，即無法有效降低喪親家屬複雜性哀傷。其他次群組分析中，介入時長「8周內(含8周)」、「8周以上」無分組差異；介入後評值時間於「介入後」、「後續追蹤」無分組差異。
結論：哀傷介入措施可有效降低喪親家屬複雜性哀傷，通過統合分析及次群組分析，可以分辨如何執行有效降低複雜性哀傷之哀傷介入措施，並且將分析之結果以相對經濟有效的方法達成一致性介入，形成臨床實務實證建議作為未來臨床之參考。

The bereaved members may continue to be impacted by strong and painful emotions, and sometimes grief will develop into complicated grief. It is advisable to identify needs early and refer to further professional assistance. The present systematic review and meta-analysis aimed to find out that the relationship between the type of grief interventions, the length of intervention time, the evaluation time after intervention, the background of the therapist and training details with the reduction of complicated grief. And combined with the results of meta-analysis to form clinical recommendations. A total of 10 studies were included for meta-analysis. The overall effect size of 10 studies was -1.45 (95% CI was -2.11, -0.79; Z=4.3; p<0.00001), showing that grief interventions can help reduce the complicated grief of bereaved families. Through meta-analysis and subgroup analysis, it is possible to identify how to implement grief intervention effectively and cost-effective in reducing complicated grief. And also forming some empirical recommendations as a reference for future clinical practice.
