隨著全球失智者人數逐年增加,迷路與走失成為失智者常見且具高風險性的認知功能障礙問題,易導致患者跌倒、骨折等傷害,亦對家庭照顧者造成嚴重生心理負擔與家庭困擾,進而加重社會資源消耗。然而,過去的研究多聚焦於科技應用,較忽略家庭照個者的教育。因此,本研究目的為:(1)發展本土化的失智症迷路與走失教育介入策略手冊;(2)依據失智者安全距離、活動能力及過去迷路經驗等迷路風險特徵,提供失智者與家庭照顧者本土化且量身訂做的失智症迷路與走失教育策略,並初探策略對失智者迷路的次數及家庭照顧者困擾改善成效。
本研究採二階段設計。第一階段以文獻查證為基礎,建構《失智症迷路與走失教育介入策略手冊》,並透過修正式德爾菲法建立專家效度,隨後邀請第一線社區實務醫療人員進行適用性評值。結果顯示,手冊內各項策略在執行可行性與困難度評估上,均有超過半數的社區實務醫療人員認為「大部分可行」且「尚可做到」,顯示策略內容具備高度可行性與中低程度的執行難度,適合用於照顧失智者的臨床情境中。
第二階段研究乃基於第一階段所發展之「迷路手冊」內容,根據失智者的安全活動距離、外出能力與風險因素,進一步重新整理並彙整策略內容,設計出更貼合家庭照顧者實際需求的量身訂做教育手冊。此階段共招募18組家庭,進行為期兩個月的單組前後測介入及評估介入後回饋。結果顯示,介入後失智者之實際迷路次數顯著下降(Z = -4.001,p = 0.000),呈現顯著成效;然而,在照顧者照顧迷路失智者困擾程度方面,雖介入後困擾有所下降,差異未達統計顯著(Z = -1.947,p = 0.052),且參與者回饋顯示策略內容具有高度契合性,其中44%認為策略「符合需求」,56%表示「完全符合需求」。
本研究結果支持該介入策略具備高度實務可行性與接受度,未來可作為臨床與社區層面推廣失智者迷路預防教育,並為後續大規模介入研究提供改善之基礎依據。
Getting lost and missing incidents are common in persons with dementia and often result in physical harm and family distress. This study developed and evaluated a locally tailored educational manual to prevent such incidents and decrease family distress. The results showed high feasibility and a significant reduction in the frequency of getting lost. Although caregiver distress showed a decreasing trend after the intervention, the change was not statistically significant. Nevertheless, participant feedback indicated that the strategies were highly relevant and met their practical caregiving needs.
