心率變異性是一種評估心率變化的指標,它也反映出自律神經的平衡狀態。健康人通常是高心率變異性,因此低心率變異性被視為疾病發作或自律神經功能障礙的警訊。新生兒敗血症經研究證實前期有異常心率的特性,因此檢測心率異常對於發現敗血症至關重要,此心率異常是指低心率變異性低、瞬間心率減速等異常現象。然而臨床上觀察心率異常性的方法並不直觀且信息單一。例如:難以通過平均心率監控來區分已證實的敗血症和臨床敗血症。而R-R 間期雖可以區分已證實的敗血症和臨床敗血症但僅提供變異分佈信息。鑒於此問題,我們提出新的可視化工具來協助臨床醫生檢測疑似假陽性的新生兒敗血症。該可視化工具以更直觀且清楚來提供平均心率、變異程度與分布等信息還有提供異常事件的評估指標,這些功能有助臨床人員檢測可能的假陽性敗血症並且提醒可縮短抗生素治療時間。
Heart rate variability (HRV) reflects the balance of the autonomic nervous system and is usually high in healthy individuals. As such, low HRV indicates disease onset or autonomic nervous system dysfunction. Low HRV is also characteristic of early neonatal sepsis. Crucial for diagnosing sepsis is the accurate detection of heart rate abnormalities. In this study, heart rate abnormality included low HRV, transient heart rate deceleration, and other abnormal phenomena. The current methods of clinical observation of abnormalities are not necessarily intuitive or informative. For example, differentiation between proven sepsis and culture-negative sepsis is difficult by solely observing the average heart rate on a bedside monitor, whereby the line graph of the R-R interval is easy to distinguish but only provides information on the distribution of variation. Given the above, we proposed a new visualization tool to help clinicians in the detection of suspected false-positive neonatal sepsis. Our visualization tool is more intuitive and accurate in providing information such as average heart rate, variability, and distribution. In addition, we determined evaluation indicators of abnormal events. These functions will help clinicians to detect possible false-positive sepsis and alert them to shorten unnecessary antibiotic treatment.
