本研究透過社群大數據,了解民眾在網路上針對糖尿病之討論趨勢、內容及討論之情緒態度,並以文字探勘之關聯規則與集群分析進一步探索民眾較常提及之討論內容。首先運用Opview社群媒體觀測平台,蒐集文本資料。確認資料品質後以R語言針對在網路上討論概況進行分析,再針對討論度最高之前三個面向透過自然語言處理過程,分別斷詞、計算詞頻、繪製文字雲、關聯分析、集群分析。研究結果由討論區、社群網站、問答網站等所收集約兩萬筆資料顯示,在糖尿病中討論度最高之前三個面向為飲食(33.2%)、生活調適(21.2%)、避免併發症(15.6%)。糖尿病於各面向討論之態度趨向負面,其中又以避免併發症(0.13)、運動(0.25)為討論時最負面之面向。細探討論度最高之前三個面向受到熱門討論之話題:飲食的部分主要是偏方、特殊飲食療法、中醫;生活調適的部份為患者的調適與家屬所受的影響、經濟方面的壓力;而在避免併發症的部份則是性功能障礙、神經病變、腎病變、視網膜病變等話題。除此之外,透過文字探勘結果發現,性功能障礙亦是民眾非常關心的重點。本研究透過由民眾在網路上自發性的發文、留言內容,找出面對糖尿病時的問題,發現在臨床醫學所強調之重點,與民眾真正在意的內容有所差異:運動在醫學上是非常強調的,對於民眾來說卻是最不關心的;而在性功能的部份是民眾最在乎的,針對這部分的衛教卻似乎有些薄弱;而在臨床上強調與民眾都非常關心的,為身體肥胖的問題。根據現有研究發現,以及以病人為中心的照護指標,建議糖尿病照護實務上可以多強調像是性功能、肥胖等民眾較在意之重點,以主題性的方式加強衛教內容。而在研究上因為本研究之資料來源蒐集範圍有限,在關鍵字設定上以AADE糖尿病自我照護行為管理為主,未來研究若在資料源的權限能夠取得更長的時間範圍或是針對文字探勘嘗試不同分析方法,並建議可以針對臨床藥物或不同疾病進行研究。
The aim of this study was to analyze the contents of people's discussions about the Diabetes Mellitus in the internet. We used the OpView social media platform to collect Social Listening Data, and analyze by the Text Mining techniques. We used the data which was collected in the period of June, July, August in 2017 and to discover what people talking about in the internet including the trend, the emotional attitudes, and the contents. The results show that Diet (33.2%), Lifestyle Adjustment (21.2%), and Avoidance of Complications (15.6%) are the top three themes which people often talked about. The results show that sexual function and obesity are the problems people most concerned, but which the clinical medicine seldom emphasized. We suggest that according to the results and patient-centered indicators, the Medical professional can consider to emphasize more about what people really care about, and to strengthen the contents of the hygiene education in the Diabetes Mellitus.
