目的:過去已對先天性畸形和變形患童的家長進行了心理適應的探討,發現腦心智功能之執行功能的動機與計畫、自我覺察力、低效能評價因素,皆與親職照顧壓力之因素呈顯著負相關;執行功能與創傷後成長分數呈顯著正相關。因應策略與親職照顧壓力呈顯著負相關。因此執行功能和因應策略為個體重要的心理資源,並在緩解多指症孩童家長的親職照護壓力與促進正向心理改變上具重要角色,這表示家長能運用優勢之的腦心智功能,協助自己啟動因應並尋求可使用的支持,有較佳的適應歷程。但過去研究並未深入討論這些策略與技巧如何轉化為有效疾病適應。本研究希望能夠針對復原力與患童家長疾病適應能力的關係進行討論,並考量不同就醫歷程的階段,發展能協助家長成功因應疾病之心理架構。
方法:本研究通過人體試驗委員會審核,編號 B-BR-107-004,於南部某醫學中心資深小兒骨科專科醫師之門診,採立意取樣,轉介8名肌肉骨骼系統先天性畸形和變形兒童家長作為研究參與者,4名進入或完成醫療處置,4名等待醫療處置。於隱蔽空間內個別進行以肌肉骨骼系統先天性畸形和變形兒童家長復原力與就醫歷程為主題之半結構式深度訪談,以建構主義的紮根理論進行質性分析。
結果:8份文本資料中,共標定出1469筆意義單元(全距為169至219)。以建構主義的紮根理論分析後,萃取出710筆概念化、276筆次範疇、32筆內涵、10個面向、以及3大核心主軸分別為復原力軸、就醫經驗軸及就醫歷程軸,其中「復原力軸」軸有五個主要面向,分別為韌性、保護網、因應能力、控制感、價值觀系統。「就醫經驗」軸有兩個主要面向,分別為客觀流程與主觀經驗兩個主要面向。「就醫歷程軸」有三個面向:被動得知、主動尋求、問題解決。
討論:復原力作為研究主題,在紮根理論下重新討論後,發現會更多的範疇包含於Conner-Davidson的心理復原力定義,這可能受到學術派典、疾病性質及文化習俗之影響。此外,同時使用就醫歷程軸與復原力軸檢視同一意義單元,能觀察到復原力不同面向與就醫歷程產生多種組合,若能在歷程下辨識優勢能力,對於家長可能有較佳的疾病適應歷程。
Objective: Previous research has investigated the psychological adaptation of parents of children with congenital malformations and deformities. However, earlier studies have not thoroughly discussed how these strategies could translate into effective disease adaptation. This research aims to discuss the relationship between resilience and the disease adaptation abilities of parents of children with health issues, considering different stages of the medical treatment process, and to develop a psychological framework that can assist parents in coping successfully with the disease.
Method: This study uses purposive sampling from the outpatient clinic of a senior pediatric orthopedic specialist in a medical center in southern Taiwan. It involves eight parents of children with Congenital Malformations and Deformities of the Musculoskeletal System—four undergoing or having completed medical treatment, and four waiting for medical treatment. Semi-structured in-depth interviews were conducted in a private setting, focusing on the resilience of the parents and their treatment experiences. The interviews were qualitatively analyzed using constructivist grounded theory.
Results: From eight text data, a total of 1469 units (169 to 219 for full distance) were identified. After analysis using constructivist grounded theory, 710 conceptualizations, 276 meaning, 32 themes, 10 aspects, and 3 main axials (resilience, medical experience, and medical process) were extracted. The 'Resilience' axis had five aspects: resilience, support network, coping ability, sense of control, and value system. The 'Medical Experience' axis had two aspects: objective processes and subjective experiences. The 'Medical-Treatment Process' axis had three aspects: passive awareness, active seeking, and problem-solving.
Discussion: Resilience as a research subject was found to encompass more categories than defined by Conner-Davidson’s psychological resilience. Additionally, using both the treatment process and resilience axials to review the same meaning units showed that different aspects of resilience could combine in various ways with the medical process. Identifying strengths during the process may lead to better disease adaptation outcomes for parents.
