驅動不同步(Trigger Asynchrony)是在病人使用呼吸器輔助呼吸時發生的一種現象。在醫療領域,辨認它是重要的。至今,醫師、治療師及護士必須以人工觀察方式來辨認驅動不同步。開發自動辨識方法可節省非常多的人力成本及勞力。本論文開發了三種主要方法自動辨認驅動不同步。此三種演算法是由不連續小波轉換、多層認知類神經網路(Multilayer Perceptron Neural Network)、局部最小值偵測及門檻值的不同組合而發展出的。其辨識正確率會和人眼辨識的正確率做比較。為了能實際運用,驅動不同步的偵測必須要建置在健康照護儀器上。因此也在此介紹已建置好的家用看護盒。
在居家看護的應用上,生理訊號監控是一個關鍵性的功能。在最近幾年中,在不同的環境中執行各種不同的生理訊號監測的不同儀器被開發出來以協助健康看護。然而,僅是監測無法符合各種情境的需要。因此,病人及照護中心的病人需要一些日常健康看護相關的互動,如用藥提醒、居家訪視安排等…。基於這個考量,本論文提出了一個提供健康監控及適用不用情境的以達到健康管理目標的家庭健康照護盒。為能能適應不同情境,一個訊息嵌入機制也在此論文被介紹。一些家庭健康照護盒中的技術也在此論文會介紹。
驅動不同步辨識機制可被建置在家庭健康照護盒的監控與分析及警訊單元。因此在家中發生的驅動不同步能被自動辨識,且家庭健康照護盒能通知醫生、治療師及護士此狀況。它能省下大量的人力、病床及經費。
Trigger asynchrony is a phenomenon which occurs when the patient breathes with the support of ventilator. The recognition of it is important in medical area. Up to now, doctors, therapists, and nurses must recognize the trigger asynchrony by manual observing. To develop an automatic recognition method can save a lot of human cost and effort. This thesis develops three main methods to recognize the trigger asynchrony automatically. The three algorithms are developed by different methods of combination of discrete wavelet transform, multilayer perceptron neural network, the local minimum detection and thresholds. The recognition accuracy of algorithm is compared with the manual recognition accuracy. For practical usage, the trigger asynchrony recognition method should be implemented in health care device. So a home health care box implemented is introduced here.
Physiological signal monitoring is a key function in the application of home health care. In recent years, various devices for physiological signal monitoring in different environments were developed for assisting health care. However, monitoring alone can’t fit the needs of many scenarios. As such, it is often that the patient and employees of the care center requires some daily healthcare related interaction, such as medicine taking reminder, home visiting arrangement, etc. . Based on this consideration, this paper has presented a home health care box which provides health monitor and fits a variety of scenarios to achieve health management. In order for the home health box to be able to adapt for different scenarios, a message embedding mechanism is also presented in this paper. Also, some technologies in the home health care box like multi-thread with key-polling, socket scheduling will be presented in this paper.
The trigger asynchrony recognition mechanism can be implemented in the monitor and analysis and alarm unit on the home health care box. Thus the trigger asynchrony occurring at home can be recognized automatically and the home health care box can notify the doctors, therapists, and nurses the situation. It can save a lot of human effort, hospital beds, and money.
