研究背景:近年來在臺工作的外籍家庭看護工人數遽增,足見家庭對外籍看護工的仰賴需求之龐大。政府於 2018 年 12 月 1 日起分三階段推動「擴大外籍看護工家庭使用喘息服務」計畫,針對聘僱外籍家庭看護工之被看護者家庭正式納入喘息服務,以解決被看護者在外籍家庭看護工休假期間的家庭照顧需求及兼顧外籍家庭看護工的休假權益。然而,政策上路以來,是類家庭對於使用喘息服務的意願仍然偏低。
研究目的:本研究目的為探討外籍看護工家庭對喘息服務的使用情形,並分析影響其使用的重要相關因素。
研究方法:採橫斷性問卷調查研究,以 Andersen 健康服務利用行為模式為基礎並編訂分類,針對高雄地區聘僱外籍看護工的家庭為對象,以立意取樣親訪說明方式完成118戶問卷。以SPSS17版統計軟體進行分析,描述性統計分析以次數分配、百分比、平均值、標準差;推論性統計以獨立樣本 T 檢定、卡方檢定、羅吉斯迴歸分析(Logistic Regression analysis)。研究結果:發現 118 戶中目前有使用過的共 37 戶(31.36%),未來將會選擇使用者共102 戶(86.44%)。而影響外籍看護工家庭使用喘息服務的顯著影響因素為:
一、照顧者前傾因素:家庭主要照顧者性別;外籍看護工:性別、目前每月休假天數、每日工作時間、家人協助照顧時數、需要半夜服務;被照顧者:居住狀況。
二、照顧者需求因素:被照顧者:長照類型、長照等級、巴氏量表、失智症狀;家庭主要照顧者:生理需求、心理情緒壓力需求。
三、照顧者使能因素:1966 專線;喘息服務的認知、訊息來源管道、未使用原因、規律與計畫性使用;對喘息服務的看法。
以羅吉斯迴歸分析影響外籍看護工家庭使用喘息服務的重要預測因子包含五大方面:(一)外籍看護工的每日工作時間及是否需要半夜服務;(二)家庭主要照顧者的性別;(三)被照顧者的居住狀況、失智症狀、長照類型;(四)1966 專線;(五)對喘息服務的看法(可利用性)、對喘息服務的看法(障礙)。
結論:「擴大外籍看護工家庭使用喘息服務」計畫,重點在外籍看護工休假時的人力協助,故先要有外籍看護工的「休假」為前提,後續才有「決定權者」是否使用喘息服務。本研究並深入瞭解外籍家庭看護工「休假的意願」、「休假的問題」、「休閒的活動」,並找出使用喘息服務的「決定權者」其真正「考量的因素」為何?透過此兩個面向再深入精髓探研,本研究結果對於推動「擴大外籍看護工家庭使用喘息服務」計畫政策,將更具有實務上的參考價值。
The purpose of this study is to explore the use of respite services by foreign care workers' families and analyze the relevant factors that affect their use of respite services.Based on the Andersen Health Behavioral model , targeting families in Kaohsiung who hired foreign care workers, and completing the 118 questionnaires in the form of Purposive Sampling.The results of the study have a total of 19 significant factors affecting the use of respite services in foreign care workers’ families: 7 Caregiver's predisposing factors; 6 Caregiver's need factors; 6 Caregiver's enabling factors. There are a total of 8 important predictors that affect the use of respite services by foreign care workers' families. This research also deeply explores the "willingness to take vacations", "the problem of vacations", and "leisure activities" of foreign family care workers, and finds out the real "factors for consideration" of the "decision-makers" who use respite services. The above results will provide important references for policy implementation.
