目的:過去研究指出醫療工作具備高專業性且高挑戰性,再加上專業助人者的工作特性,臨床心理師等相關心理衛生人員易因情緒耗竭、工作壓力而導致工作疲匱(burn out),影響其工作滿意度,甚至產生離職意願。過去針對臨床心理師工作滿意度之研究,多以工作壓力面向進行探討,較少針對專業正向發展的面相進行討論。而研究指出心理衛生職業人員之專業認同(profession identity)和專業自主性(professional autonomy)分別都與工作滿意度(job satisfaction)呈正相關。然目前探討台灣臨床心理師的專業自主性、專業認同的研究甚少,故本研究旨在了解醫療領域之臨床心理師專業自主性、專業認同之內涵。其次,探討專業自主性、專業認同,以及工作滿意度之關係。最後,探討專業自主性、專業認同對工作滿意度的預測效果。
方法:本研究以問卷調查法進行。對領有臨床心理師執照證照且現(曾)於台灣地區醫療院所執業之臨床心理師為研究對象,共納入同意參與研究之臨床心理師295名。研究工具包含:基本資料表、臨床心理師專業認同量表、臨床心理師專業自主性量表,以及臨床心理師工作滿意度量表,運用Google表單製作網路問卷。以描述統計、因素分析、Cronbach信度分析、Pearson相關,以及結構方程模式(structural equation modeling, SEM)等進行資料分析,並檢驗其關係。
結果:以標準化分數檢驗,刪除三個量表總分之離群值後,有效資料為289名。臨床心理師專業認同量表、臨床心理師專業自主性量表,以及臨床心理師工作滿意度量表皆經內部一致性信度、專家內容效度及建構效度之檢驗,具備適中信效度。臨床心理師之專業自主性、專業認同對工作滿意度皆有顯著預測效果(β=.72,p<.001;β=.56,p<.001)。結構方程模式之結果顯示,專業認同透過專業自主性預測工作滿意度,整體預測模式對工作滿意度的總解釋量為55.3%。經信賴區間法(Bias-corrected bootstrap method)檢驗,專業自主性為專業認同與工作滿意度之中介變項。
討論:本研究結果顯示臨床心理師之專業認同與專業自主性皆可預測工作滿意度,然專業自主性的直接預測效果遠高於專業認同,且專業自主性為專業認同與工作滿意度之中介變項。依據此研究結果,研究者建議醫療團隊之臨床心理師爭取合宜的臨床專業自主性,在醫療團隊展現其重要性與價值,以增進臨床心理師整體之工作滿意度,並提升醫療照護品質。
The objective of this study is to investing the predictive effect of professional identity and professional autonomy on job satisfaction of clinical psychologists in hospitals. There are 295 clinical psychologists agreed to participate in the study. The demographic data and work-related information were collected. Then all participants were asked to complete the professional identity, professional autonomy and job satisfaction scales. Correlation analysis and structural equation modeling (SEM) were conducted to assess the relationships among the data. The results of the study on a prediction model for predicting job satisfaction by self-reported profession identity and profession autonomy. Both profession identity and profession autonomy had positive effects on job satisfaction of clinical psychologists in hospitals, and professional autonomy was the stronger influential factor than profession identity. Hence, strengthening the clinical psychologists self-reported professional autonomy and professional identity may be helpful for increasing their job satisfaction.
