  • 成大醫院癌症病人生活品質研究平台[2011/05~迄今(2018/07)]的價值主張是:壹、健保永續經營。貳、癌症病人活得久、且活得好。為呼應上述的價值主張,此平台收集的資料的用途為:1. 供健保決策用的”成本效用分析(cost-utility analysis, CUA)”,以健康人年(QALY, quality-adjusted life year)為比較單位。2. 收集分析已接受治療病人的自我陳述生活品質資料,即病人心裡的”感覺/舒服”程度”。做為實證醫學的一部份,供病人參與”醫病共享決策”時的參考。
    成大醫院癌症病人生活品質研究平台是一個「知識型組織」。迄今已七年多。我們除了想探索此「知識型組織」的建構歷程。達到”知其然,到知其所以然”之外;也想作為他人日後建構「知識型組織」的借鏡。
    「價值主張(value proposition)」是敘述企業所推出的產品或服務,可以解決某一潛在目標顧客的痛點。希望這樣的陳述能引起部分潛在目標顧客的共鳴,取得市場占有率。差異化行銷,走向藍海市場。
    “顧客參與”的定義是「顧客在其動機驅使下,針對特定品牌或特定企業,所表現之購買行為以外的其它行為」。這當中有”顧客主動參與”、”組織公民行為”、”顧客共同創造”、和”取得社會連結”。
    “價值共創”的概念是來自於”價值基礎策略觀點”。最基本的價值共創概念是所有利害關係者(stakeholder)能透過資源共享、整合的活動,以擴大共同的價值;於此同時也增加彼此各自的價值、實現各自所需之利益。
    我們將結合”價值主張”,”價值共創理論”,和”顧客參與”。並對參與此平台的成員(醫師或研究人員)進行訪談,來剖析成大醫院癌症病人生活品質研究平台的建構過程。探索和尋求下列問題。
    1. 探討此知識型組織的成員是如何逐漸由少到多,由疏到密結合在一起?
    2. 探討這個平台所從事的活動,是否真能滿足其高舉的價值主張?即:健保永續經營;和提供醫病共享決策的實證資料。
    3. 探討這平台未來後續發展的方向為何呢?
    用前述理論架構,穿插在平台建構的四個時期。每個時期都會盡量用「背景」、「主要活動」、「遭遇困難」、「如何克服」、和「反思」等五個段落來敘述成大醫院癌症病人生活品質研究平台的建構歷程。
    結論:價值主張是核心,主張要明確,且要不斷地宣揚價值主張,鞏固成員的信念。透過價值共創四大核心活動(利誘、尊重、互動、支援)中的利誘找到對的共創者。藉著”互動”和顧客主動參與、組織的公民行為、顧客共同創造、和取得社會連結的概念是由小而大,由簡而繁。以戰養戰。最後會產生一批願意奉獻的同好。

    Value propositions raised by the research of quality of life of cancer patient platform of National Cheng Kung University Hospital are as following: sustainability of national health insurance; and live longer and live better of cancer patient. The achievements have been reached by the platform are cost-utility analysis and shared decision making of cancer patients.
    Quality of life (QOL) was one of elements of patient-reported outcomes. There are two major approaches to assessing individual's quality of life. One is based on economics and the other one is based on psychometrics. As a result, the first one provides a single aggregate index to represent individual's QoL. On the other hand, the second one provides multiple scores on diverse QoL dimensions. The representative questionnaire for economic is the European Quality of Life Scale (EQ-5D), for psychometric is the World Health Organization Quality of Life Questionnaire (WHOQOL). The individual’s utility value is derived from EQ-5D, and quality-adjusted life year (QALY) is derived from utility value and survival year. QALY is a symbol that taking the survival and quality of life into consideration in the same time. In combination with other databases, the cost-effectiveness ration CER, life-time cost/QALY) was derived also. Up to now, CER of 19 Taiwan’ cancer were done already. What we done mentioned above is the basis for incremental cost effectiveness ration (ICER). ICER is a tool for the justified whether or not the NHRI will cover the new drugs, new techniques, or new cancer screening tool when comparing with existed already one.
    Up to now (2011/05~2018/06), there were ten types of cancer, 13,145 patients, and 50,144 repeated measurements had been collected. Two papers regarding the advanced lung cancer patients with three different treatment regimens who received WHOQOL questionnaire revealed different responses in four domains (physiology, psychology, social, and environment). Results of above two papers could be as a reference for the lung cancer new comers to choose drug regimens.
    This is a qualitative research we took the Quality of life research platform of National Cheng Kung University Hospital as an example. In combination with value proposition, customer engagement, and value co-creation perspective, and put the all data together, the time-line of platform was explored by different stages of value co-creation. With the lung cancer as an example, the analytic structures were as following: background, major activities, difficulty encountered, how to solve the problem, and reflection of each stage.
    The results of our study, first of all, four major keystones (DART, dialogue, access, risk, transparency) of value co-creation were applied repeated in different stages. Secondarily, value proposition of platform should be raised positively and repeatedly. Thirdly, reconstruction of resource should be done beforehand to attract potential participants. And finally the key person had the responsibility to inspire the crew when the depressed atmosphere noted. Published papers were the most import inspiration to the crew and could expand our influence to the society
