  • 不適當的病護比與工作負荷都會造成護理人員疲憊及工作滿意度下降,降低照護品質。因此,長期累積不適當的工作負荷,若未得到適當的紓解,再加上工作需求不能滿足,則極易導致疲憊及不滿的情緒。若能了解護理人員工作負荷與工作滿意度的關係,將有助於協助護理業務之推動,並進而提升護理人員照護能力、激發成就感、並增加滿意度。
    本研究目的是探討台灣護理人員工作負荷與護理工作滿意度之關係,為描述性相關性研究。使用次級資料分析,資料庫來源為衛生署2008 - 2011年科技研究計畫「護理人力資源與病患成果」計劃之資料庫,分析35家醫院,共117個護理單位與1846位護理人員參與研究。自變項為護理人員基本屬性(基本資料、教育程度、工作經歷)、護理單位工作飽和度、病床週轉率、每日護理時數、病護比、技術組合比,依變項為護理人員滿意度,以回歸分析探討其關係。在考量基本屬性對工作負荷的影響後,發現臨床工作負荷N人員技術組合比例是可以預測護理人員的滿意度,護理單位工作負荷中之N人員技術組合比例與滿意度呈負相關(- 0.244, p < 0.05),即單位上班之 N人員比例越高,滿意度越低,解釋變異量為25.5%。建議單位主管在排班或分配三班照護之護理人力,要適當考量平均分配人員組合比例,讓臨床護理人員在工作上遇到問題時,有層級較高之人員可以詢問及互相幫忙,以增進護理工作滿意度。

    Nurses are the caregivers at the front line. They are required to possess the professional knowledge, the ability to combine care with other team members, and to deal with busy routine work everyday. We aimed to explore the relationships between the nursing workload and the job satisfaction in Taiwan, and, furthermore, to provide hospital managers a guide to create a positive and pleasant environment in nursing and to improve job satisfaction among nurses.
    This was a descriptive correlation study according to secondary data analysis from "Nursing Utilization of Resources, Staffing and Environment (NURSE) on outcomes study" funded by Department of Health, Executive Yuan, Taiwan through 2008 to 2011. One thousand eight hundred and forty six nurses in 35 hospitals were included. Regression analysis was applied to elucidate the relationship between job satisfaction and other variables, including the basic profile, saturation of each unit, transferring rate, ratio of patient to nurses, and technology portfolio.
    There was negative correlation between nursing units in the workload of the technical ratio N skill mix and satisfaction (- 0.244, p < .05) , and nursing units that the higher the proportion of N skill mix, lower satisfaction, 25.5% of variance. N skill mix of technologies can be predicted proportion of nursing staff satisfaction
    We suggest that the appropriate nursing schedule should correspond with the work experience, educational level and technology portfolio of the average distribution. The job satisfaction can therefore be improved if the junior nurses can handle problems with facilitation by senior nurses who are more experienced and have higher level of skill mix of technologies.
