癌症是全球人類、亦為台灣國人的主要死因,發現生命意義有助於癌症末期病人善終,然而目前護理人員協助病人發現生命意義的靈性照顧能力仍有努力精進的空間,也少有研究進行探討。本研究的目的為探討癌症末期病人的生命意義,以詮釋學為研究方法論,採立意選樣選取2位癌症末期病人,以深度訪談進行資料收集,收案時間為2008年9月至2009年3月。研究結果發現癌症末期病人的生命意義為:「犧牲奉獻,只為能守護家,傳宗接代有好子孫」、「盼同理,求存活,忍屈受辱;被傾聽,惦記責任,忘卻病痛」、「信靠神明,險難中更感至高者臨在」、「漸無能,恐拖累,接納需依賴他人,學習被愛,真情關愛暖在心」、「怨嘆歹命中,參透為何受苦,只好認命,笑看苦樂」、「為苦所惑,渴求擺脫;正向思考,體會苦難中藏寶藏,促發生命躍升」、「陷於逆境,渴盼轉機」、「生死自然,前往彼岸,一生足跡遺留人間,雖死猶在」。研究結果可作為護理人員的背景知識,增加對癌症末期病人生命意義的瞭解,透過協助病人發現生命意義方式,為病人提供靈性照顧,以期達到病人善終、家屬善別、護理人員善生。
Cancer was an important cause of death in Taiwan and the whole world. Discoverying the meaning of life promotes good death. But clinical nurses often cannot meet the needs of terminally ill cancer patients.and there were few research about it. The purpose of this study was to explore the meaning of life of the terminally ill cancer patients. Purposive sampling was used in this study. The methodology was Hermeneutics. The approach of data collection was from recall by in-depth interviewing from September 2008 to March 2009. Eight constitutive patterns were merged: “sacrifice and dedication for the sake of one's family and offspring”, “wishing for empathy ; striving to survive, enduring shame and humiliation ; needing to be listened, concerned about responsibilities, being relieved of suffering from illness”, “believing in some sort of deity ; further perceiving being with the Higher Being in life's difficulties and hazards”, “afraid of becoming a burden due to one's incompetence ; accepting one's need to depend on others ; leaning to be loved and feeling the warmth of love and affection”, “lamenting one's ill fate, then realizing the reason for suffering ; finally accepting one's destiny and rising above one's mortality”, “confused by one's suffering and yearning for relief ; using positive thinking to realize that there is always hidden treasure amid suffering , and to promote the enrichment of one's life”, “trapped in one's adversities and embracing hope”, “accepting the natural course of life and death, leaving for the afterlife peacefully ; realizing that one will never die with one's memorable legacy”. From the result, we had better understanding of the meaning of life of the terminally ill cancer patients. To enhance professional competence of spiritual care.
