本研究以儒家文化為背景，探討廣東與臺灣兩地的精神病友家屬照顧者和助人者的照護負荷以及他們的自我角色知覺。中外目前有關精神病友家屬與助人者的照護負荷研究，通常是採個別分立的觀點進行表象的統計資料描述，缺乏同時將兩類照顧者進行充分對比，亦缺少足夠的文化反思及理論觀。因此，本研究以儒家的「積極義務」、「消極義務」、「義命合一」及「義命分立」之文化視域為出發，透過對廣東與臺灣兩地精神醫療照護制度的差異比較，探討兩地家屬及助人者對精神病友的照顧負荷及其自我角色知覺。本研究採質性研究取向，研究方法以半結構式訪談為主，問卷資料蒐集為輔的形式進行。研究結果發現，兩地家屬和助人者都是受到「積極義務」的驅使而對精神病友進行照顧；而在心理負荷調適與自我角色知覺上，家屬通常是以「義命合一」的自我調適觀接受現狀並持續付出；助人者則是在「義命分立」的前提下會試圖追求改變。另外在照顧負荷差異上，兩地家屬都表示自己的諸多生活層面確實因為照顧精神病友而被影響，而廣東的受訪家屬還表示有經濟方面的負荷。而就助人機構類型區分，則發現公立機構存在工作量過大和醫病關係的問題，私人機構則存在資源配置不足的問題。最後在提供照顧者心理處遇措施的需求上，家屬們傾向不會主動尋求協助，而助人者則會希望能獲得心靈與物質兼具的多元化需求。本文最後提出研究限制與建議，期能讓社會大眾更瞭解精神病友照顧者的心路歷程，並促發學界與實務界對此議題的重視與省思。

This study explored the burden of care and self-role perception of family caregivers and health professionals of psychiatric patients in Guangdong and Taiwan in the context of “Yi” and “Ming” from Confucian culture. Though qualitative research, the first study showed that family caregivers and health professionals in Guangdong were both suffered from the psychiatric patient's condition, and the family caregivers also had problem in financial difficulties; the second study also got the conclusion that family caregivers in Taiwan endured the burden from the psychiatric patient's condition as well as study 1, and the burden of health professional in Taiwan were different due to the different types of organizations, corresponding to the difference in workload and insufficient resources. In the end, the research limitations and suggestions were put forward in the hope that the public could better understood the psychological process of the caregivers of psychiatric patients, and encouraged the academics and practitioners to pay more attention to and reflect on this issue.
