本研究是在護理人力不足之前提下,規劃以複合式護理照護模式(nursing skill mix model)執行醫院一般病房護理照護,主要目的為改善一般病房護理照護人力不足現況,提昇住院病人照護品質。本研究是以南部某醫學中心神經外科、一般外科病房及血液腫瘤內科、腎臟內科病房為研究對象,探討在複合式護理照護模式下,進行住院病人及護理人員在護理服務品質、護理服務效率、複合式護理照護模式的認同度與整體服務滿意度的認知與兩者之間的差異性,且進一步探討複合式護理照護模式在護理人力運用的成本效益。
本研究結果顯示:複合式護理照護模式介入前住院病人的知覺服務品質、服務效率滿意度高於護理人員的自覺服務品質及服務效率滿意度;介入後護理人員自覺服務品質滿意度高於住院病人知覺服務品質滿意度;在護理人員對護理照護模式認同度的看法是照護模式介入前、後相差不大,但是介入後護理人員對於照護模式的看法較有差異性;整體而言,照護模式介入前住院病人對於護理照護人員提供的整體服務滿意度高於護理人員自覺服務滿意度,而在照護模式介入後,護理人員自覺整體服務滿意度高於介入前之整體服務滿意度,表示護理人員對護理照護模式的介入較有信心讓病人得到滿意。
在成本效益上,照護模式介入後神經外科護理人員每人平均逾時增加18.6分鐘,腎臟內科護理人員每人平均下降7.62分鐘,腫瘤內科護理人員每人平均下降31.9分鐘,效果最顯著;在人力成本上,因照護模式介入而配置的護理佐理員人力成本,相較若以補足應配置護理人員人力成本,顯示每月可節省308,694元之成本效益。
研究結果提供醫院經營者及護理管理者管理之參考依據。
The research planed to apply nursing skill mix model in general nursing care under the nursing manpower insufficiency. The main purpose was to improve the manpower insufficient present situation in general hospital wards and to increase the quality of nursing care. The objects of this research are neurosurgery, common surgical, oncology medicine, and the kidney medicine in the south some medical center. And discussed with the correlation and the diverseness of patients and nursing staff's acceptance and the overall degree of satisfaction in the quality of care, efficiency of care, and nursing skill mix model under the pattern in skill-mix of nursing model. And the research also surveyed the cost benefit of using nursing manpower in the Skill Mix of Nursing.
The results indicated that, patients' perceptible service quality, service efficiency satisfaction are higher than nursing staff's self-perceived service quality satisfaction before the intervention. After the intervention, nursing staff's self-perceived service quality satisfaction is higher than patients' perceptible service quality satisfaction. After the intervention, there are no differences in nursing staff's self-perceived service acceptance. But the nursing staffs have some different opinions after the intervention.
As a whole, patients' satisfaction of the service that afforded by the nursing staff is higher than nursing staff's self-perceived service quality satisfaction. But after the intervention, nursing staff's whole service quality satisfaction is higher than nursing staff's self-perceived service quality satisfaction before the intervention. It showed that patients' are satisfied with nursing staff's self-confident after the intervention of Nursing Skill Mix Model.
At the cost benefit, nursing staff in the neurosurgery department averagely increase overtime 18.6 minutes; nursing staff in the kidney medicine averagely decrease 7.62 minutes apiece; the outcome is most conspicuous in the oncology medicine, nursing staff averagely decrease 31.9 minutes apiece.
At the manpower cost, the current disposing nursing aids compared to the manpower cost all of the insufficient in nursing staff , it showed each month may save 308,694 Yuan manpower costs.
This reaserch provides nosocomial operators and nursing managers a perspective on administering.
