本研究目的是探討地區醫院護理人員的工作負荷與心理健康狀態、工作滿意度和疲潰之相關性,為描述性相關性研究。使用次級資料分析,擷取「護理工作負荷與病患成果」資料庫中地區醫院之資料,進行分析,研究樣本共20家地區醫院568位護理人員。
研究結果顯示,工作負荷中之病床週轉率、佔床率、轉出、白班、小夜班及大夜班病護比與心理健康狀態呈現正相關。年齡與護理人員心理困擾呈現負相關。婚姻狀況、教育程度、執照別、病床週轉率、年齡、護理總年資在心理健康困擾具有5.9%的解釋力。
工作負荷之小夜班的病護比與工作滿意度呈現正相關,而工作負荷之轉出及每日平均護理時數與工作滿意度呈現負相關。護理人員年齡與工作滿意度,呈現正相關。護理人員的年齡、執照別、專業能力、專業能力級別、婚姻狀況、轉出對工作滿意度具有7.1%解釋力。
工作負荷中之病床週轉率、佔床率、轉入、轉出、每日平均護理時數、大夜班病護比及三班N至N2比與疲潰程度之情緒耗竭呈現正相關。年齡、平均住院天數與疲潰程度之情緒耗竭,呈現負相關。護理工作負荷的病床週轉率、佔床率、大夜及小夜病護比與缺乏人性化,呈顯著正相關,而平均住院日,呈顯著負相關,護理人員的專業能力、病床週轉率、轉出、執照別、全職或兼任在情緒耗竭上具有5.5%解釋力,護理人員的全職或兼任對缺乏人性化上具有4.0%解釋力。期本研究結果能提供地區醫院護理行政主管在人力配置措施上之參考。
The purpose of this study was to explore nursing workload in relation to mental health, job satisfaction and burnout among regional hospital nursing staff. This study was a correlational study with secondary data analysis. Data was extracted from Nursing Utlization of Resources, Staffing and Environment on outcomes study (NURSE outcomes study). A total of 20 regional hospitals, 568 nursing staff was included in data analysis. Multiple regression was applied to investigate the relationships among studied variables.
The results of this study indicated the following:
Workload indicators, turnover rate, occupancy rate, transfer rate, day shift, patient to nurse ratio in night shift were positively correlated with psychological distress. The average age among staff nurses was negatively correlated with psychological distress. Marital status, educational level, licenses, turnover rate, age, total year of experience in nursing had 5.9% of explanatory variance on psychological distress.
Patient to nurse ratio at night shift and job satisfaction were positively correlated. Indicators on transfer rate and hours per patient day were negatively correlated job satisfaction. The average age among staff nurses, license status, professional competence, marital status, transfer rate had 7.1% explanatory variance on job satisfaction.
Workload indicators on turnover rate, occupancy rate, transfer rate, hours per patient day, patient to nurse ratio at night shift and ratio of N to N2 were positively correlated with the dimension of emotional exhaustion in burnout. Average age among staff nurses, and patient’s length of stay were negatively correlated with the dimension of emotional exhaustion in burnout. Nursing workload indicators of turnover rate, occupancy rate, and patient to nurse ratio at night shift were positively correlated with the dimension of depersonalization in burnout, while patient’s length of stay was negatively correlated with depersonalization. Nursing professional competence, turnover rate, transfer rate, license status, and full-time or part-time had 5.5% explanatory variance on emotional exhaustion. Full-time or part-time status among staff nurses had 4.0% explanatory variance on depersonalization. Nursing workload was correlated with mental health, job satisfaction and burnout among staff nurses.
