隨著世界各國預期壽命持續延長,糖尿病的患病率卻不斷增加。糖尿病對社會帶來的負擔不只有醫療支出,尚包含對於生產力的影響,以及由糖尿病引起的併發症所導致的額外治療費用和損失,也應當被考量。如何客觀的從總體經濟、社會的角度衡量疾病所帶來的損失,進而比較糖尿病發病前後的損失差異,以及各年齡層、不同國家間該疾病所造成的負擔差異。研究結果將有助於公共衛生政策的制定參考。
本研究運用失能調整人年(Disability-adjusted life years, DALYs)與人力資本法(Human Capital Approach, HCA)做為評估方法,衡量2021年糖尿病對台灣和韓國民眾健康與經濟的影響。失能調整人年為哈佛大學與世界衛生組織提出的一種衡量健康損失的方法,也是目前衡量疾病損失的主流方法之一,優點在於評估方法簡單快速且易於比較,缺點為計算上通常只設定單一死因,並不是由診間實際測量。經濟損失方面,本研究使用人力資本法,從社會角度衡量疾病的影響。
研究發現:台灣每千人健康損失男性約為7.79DALYs,女性約為5.9DALYs。南韓每千人健康損失男性約為7.13DALYs,女性約為3.69DALYs。經濟損失以人力資本法衡量。總永久性損失的部分,台灣男性為3.5億美元,佔GDP 0.048%;南韓約為5.1億美元,佔GDP比例為0.028%。總暫時性損失,台灣為0.32億美元,佔GDP 0.004%;南韓則是0.73億美元,佔GDP 0.004%。
To estimate the impact of noncommunicable diseases on the economy, this study utilizes disability-adjusted life years (DALYs) to measure health loss and applies the human capital approach (HCA) to calculate productivity loss. The findings indicate that in Taiwan, the health loss is approximately 7.79 DALYs per thousand individuals for males and 5.9 DALYs per thousand individuals for females. In South Korea, the corresponding estimations are approximately 7.13 DALYs per thousand individuals for males and 3.69 DALYs per thousand individuals for females. Total permanent productivity losses amount to 350 million USD for Taiwanese males (0.048% of GDP) and 356 million USD for Taiwanese females. In South Korea, the estimated permanent losses are 510 million USD (0.028% of GDP). Total temporary productivity losses in Taiwan are around 32 million USD (0.004% of GDP). Temporary productivity losses are approximately 73 million USD (0.004% of GDP) in South Korea.
The study reveals that the group of aged 55 to 59 years experiences the highest DALYs loss in Taiwan. Furthermore, as individuals got older, the impact of years of life lost becomes more pronounced in men compared to women, although women also experience a similar phenomenon. In Korea, the group of aged 50 to 54 years encounters the highest health effects or DALYs loss. These findings shed light on the policy implications on the prevention of noncommunicable diseases in Taiwan and South Korea.
