幽門桿菌在感染宿主初期,利用BabA蛋白質與胃上皮細胞上的Lewis b血型抗原相互結合,藉此附著能力抵抗宿主胃部蠕動與排空,使菌體能進入胃黏膜層生長、繁殖與群居。相關研究中發現babA基因與babB基因約有10-4的基因重組機率,此外,不同菌株babA與babB基因具有多樣性表現,且於病人胃環境表現的菌株群也有所差異,此結果暗示著幽門桿菌面對宿主環境變化時具有高度適應性。本研究針對台灣babA與babB基因重組的多樣性進行分析,嘗試利用分析結果找出台灣菌株babA/B基因型的不同型態與疾病間的關聯,且進一步分析此關聯的可能造成原因。針對109株之臨床菌株分析,發現台灣菌株babA/B locus呈現四型基因型分佈且依菌株表現比例多寡排序為A B (39/109,35.8%),AB B (34/109,31.2%),AB AB (31/109,28.4%),和A AB (5/109,4.6%) (Figure 2),其中以A B型佔35.8%最多。此外亦發現AB AB型態為台灣特有型且胃癌相關 (p = 0.01),經胃癌相關可能因子分析後發現AB AB型態與急性發炎相關 (p ≦ 0.05),而與其黏附功能及癌前病變發生無關。
Helicobacter pylori exploit blood group antigen-binding adhesin (BabA) to interact with Lewis b to establish persistent colonization on gastric epithelium cell. Previous studies have shown that babA can be replaced by babB, an uncharacterized outer membrane protein that is closely related to babA. Many different genotypes are found in the clinical strains and associated with different adhesion ability. Whether different genotypes of H. pylori are related to the clinical outcome remains to be answered. The aims of my project were to understand the gene profiles of babA and babB presented in our clinical strains, the correlation between gene profiles and gastric disease. A total of 109 strains collected from NCKU hospital were used to analyze different gene profiles were analyzed by PCR. The A B gene profile (39/109,35.8%) was more dominant than other gene profiles, AB B (34/109,31.2%), AB AB (31/109,28.4%), A AB (5/109,4.6%). The AB AB gene profile type is specific type in Taiwan, and correlation with gastric cancer (p = 0.01). The correlation between AB AB gene profile type and gastric cancer was gone through acute inflammatory (P ≦ 0.05) not through different Leb binding activity and precancerous changes.
