背景及重要性
許多失智症患者因認知導航能力受損,常會有迷路的風險,不僅影響患者的日常生活,也給家屬和社會大眾帶來困擾;然而目前的文獻上關於探討失智患者之認路障礙現象的研究多著重於應用科技部分,因此,從家屬角度探討他們所遭遇的困境及因應經驗,對未來發展照護家庭迷路失智症患者策略或計畫具有重要意義。
研究目的
探討家庭照顧者對於有迷路風險之失智症家人的照顧困境與因應策略。
研究方法
為探索性研究設計採質性資料收集法,透過立意取樣於某醫學中心之失智症門診邀請失智症家庭照顧者且進行一對一深度訪談直到資料呈現飽和,運用半結構式訪談大綱進行訪談,收案完成後運用主題分析法進行分析。
研究結果
共訪談24位失智症家庭照顧者,根據訪談文本進行分析一共分類兩大範疇: 範疇一為面對迷路患者時遭遇之困境,歸納出四個主題,一為「對失智症識能不足」,又分為「其他家人易忽略迷路的嚴重性」、「失智家人會被汙名化」、「對可用資源了解程度不足」三個次主題;二為「身心不堪其擾」,又分為「負面心理感受」、「生活步調受影響」、「尋找時勞心勞力」、「患者配合意願低」三個次主題;三為「社會資源的幫助有限」,又分為「輔助工具不適用」、「相關機構幫助不大」兩個次主題;四為「患者配合意願低」,又分為「患者無病識感或仍具自主性」、「家庭角色關係影響患者配合度」兩個次主題。範疇二為處理應對迷路情形之策略,歸納出三個主題,一為「善用各類社會資源」,又分為「使用科技輔助工具」、「隨時備好識別信息」、「啟動社區關鍵人協助」三個次主題;二為「依據個別化需求提供照護」,又分為「順應及尊重患者意願」、「安撫與溝通」、「建立規律日常生活模式」三個次主題;三為「提供安全支援」,又分為「清晰的指示與約定」、「避免患者單獨外出」兩個次主題。
結論
本研究透過深入訪談24位失智症家庭照顧者,揭示了他們在照顧有迷路風險的失智症家人時所面臨的困境和應對策略。結果顯示家庭照顧者面臨的主要困境包括對失智症的識能不足、身心壓力巨大以及社會資源的不足;而應對迷路困境時家庭照顧者採用了多種策略,如善用各類社會資源、根據個別需求提供照護及提供安全支援。本研究結果為未來制定支持失智症患者家庭的策略和計畫提供了重要依據,強調了從家庭照顧者角度了解其困境及因應經驗的必要性,望能做為家屬及相關照護人員之參考,以提升我國在失智症者迷路相關研究與實務。
This study investigates the challenges faced by family caregivers of dementia patients at risk of getting lost and the strategies they employ. Dementia-related navigational impairments pose significant risks, affecting both patients and their families. Using an exploratory design, qualitative data were collected through in-depth interviews with 24 family caregivers. Thematic analysis revealed two main categories: challenges and coping strategies. Key challenges include insufficient awareness of dementia, physical and mental distress, limited social resources, and low patient cooperation. Coping strategies involve effective use of social resources, individualized care, and safety support. These findings highlight the urgent need for improved support systems for caregivers and suggest directions for future care strategies. This research provides valuable insights for developing more comprehensive care plans for dementia patients at risk of getting lost.
