  • 胃潰瘍是指胃黏膜受到深入黏膜下層的破壞。幽門螺旋桿菌感染、長期服用非類固醇抗發炎藥物、飲酒、壓力和吸菸是導致胃潰瘍的常見風險因子。常見治療胃潰瘍的藥物有組織胺二型受體拮抗劑與氫離子幫浦抑制劑,然而,這些藥物會有不良作用產生。因此,尋找較少或沒有不良作用的藥物來治療胃潰瘍是很重要的。甘露飲是一種傳統的複方中藥,古籍上記載甘露飲對於胃炎、喉嚨痛與口腔潰瘍具有療效,然而,目前對於甘露飲對胃潰瘍的作用仍然不清楚。因此本研究的目的為探討甘露飲對大鼠經酒精酸誘導所引起胃潰瘍的保護作用。將雄性 Spraque Dawley 大鼠隨機分成 5 組,以灌食的方式預處理甘露飲 (0, 3, 10 或 30 mg/kg) 一天一次連續五天。在第 6 天除了 Normal 組的大鼠,其餘組別的大鼠灌食酒精酸 (60% Ethanol/150 mM HCl; 1mL/200g b.w.) 以誘導胃潰瘍。1 小時後犧牲並採集胃黏膜組織,評估胃黏膜完整性、膠原蛋白、發炎相關蛋白與胃黏液分泌蛋白。甘露飲降低了潰瘍指數、促炎細胞因子(腫瘤壞死因子-α、介白素-1β與介白素-6)的含量與環氧化酶-2的蛋白表現量。除此之外,甘露飲增加了介白素-10與一氧化氮的含量、膠原蛋白與胃黏液的含量與 SRY-Box 轉錄因子 9、轉化生長因子-β1、黏液素 5 AC、黏液素 1、表皮生長因子受體、環氧化酶-1與內皮型一氧化氮合成酶的蛋白表現量。綜合以上實驗結果,甘露飲可能藉由抑制促炎細胞因子並促進膠原蛋白生成與胃黏液分泌,進而達到保護大鼠經酒精酸誘導所產生胃潰瘍的作用。

    A gastric ulcer is an injury to the gastric mucosa that goes deep into the submucosa. Helicobacter pylori infection, long-term use of non-steroidal anti-inflammatory drugs, alcohol consumption, stress, and smoking are the risk factors for gastric ulcers. Gastric ulcer is commonly treated with histamine blockers or proton pump inhibitors; however, they possess adverse effects. Therefore, finding a drug to treat gastric ulcers without adverse effects is important. Gan-Lu-Yin (GLY) is used in traditional Chinese medicine for treating gastritis, sore throat, and oral ulcers. However, the effect of GLY on treating gastric ulcers is unknown. Therefore, this study aimed to examine the protective effect of GLY on acidified ethanol-induced gastric ulcers in rats. Male Sprague Dawley rats were randomly divided into five groups. Rats were pretreated orally with GLY (0, 3, 10, or 30 mg/kg, respectively) for five days. On day 6, all the rats, except the normal group, were orally given acidified ethanol. After 1 hr, rats were killed, and gastric mucosa was collected. Gastric mucosal integrity, inflammation, collagen, and mucus-secreting protein were assessed. GLY decreased the ulcer index, tumor necrosis factor-α, interleukin (IL)-1β, IL-6 levels, and cyclooxygenase (COX)-2 expression. Moreover, GLY increased IL-10 and nitrite levels, the collagen and mucus content, and SRY-Box transcription factor 9, transforming growth factor-β1, mucin (MUC) 5 AC, MUC1, epidermal growth factor receptor, COX-1, and endothelial nitric oxide synthase expressions. In conclusion, GLY might protect the gastric mucosa from acidified ethanol-induced gastric ulcer by inhibiting inflammatory cytokines and increasing collagen and mucus secretion.
