纖維肌痛症患者長期受苦於全身性肌肉疼痛而嚴重影響生活作息,臨床研究指出纖維肌痛症患者常伴隨淺睡增加之睡眠障礙及焦慮、憂鬱等情緒問題。目前臨床上尚無確切有效治療纖維肌痛症的方式,主要原因可能與纖維肌痛症病理機轉尚不清楚有關,因此Sluka(2001)提出於大鼠腓腸肌重複施打pH4.0溶液以誘發大鼠雙側腳掌長期性的痛覺過敏現象,為了驗證此動物模型的實用性,本研究進一步檢驗此動物模型的睡眠及焦慮、憂鬱表現是否跟纖維肌痛症患者有類似的表現。研究結果顯示,大鼠被誘發疼痛後,淺睡期時數顯著增加,腳掌退縮閾值與淺睡期時數呈負相關、而與深睡期時數呈正相關。在大鼠被誘發疼痛後可記錄到進入架高型十字迷宮的開放式空間時間顯著減短,且進入開放空間之中央區域時間也縮短,糖水之攝取量顯著減少,但強迫游泳測試中不動時間則沒有差異。本研究結果證實以酸性溶液誘發的大鼠纖維肌痛症模型會有與疼痛嚴重度相關之淺睡增加現象以及高焦慮且喪失興趣之憂鬱行為產生,這些現象支持利用酸性溶液誘發長期疼痛方式適合做為纖維肌痛症之動物模型。
Fibromyalgia patients suffer from chronic widespread muscle pain. Increased light sleep and anxiety/depression disorders are major comorbidities of fibromyalgia. To date, no effective treatment on fibromyalgia in clinic may be due to poor understanding of its mechanism. Sluka (2001) introduced an animal model with induced bilateral, long-lasting hyperalgesia by repeatedly injecting pH4.0 saline into left gastrocnemius muscle to mimic fibromyalgia. To further valid the fibromyalgia model, recordings of sleep and anxiety/depression behaviors were carried out. First phase of non-rapid eye movement (NREM) sleep was significantly increased with a negative trend with paw withdrawal threshold when pain occurred. Moreover, a significant positive relationship was found between paw withdrawal threshold and duration of second phase of NREM sleep, i.e., slow-wave sleep. Hyperalgesic rats displayed significantly longer duration entering open arm of an elevated plus maze and remarkably longer duration crossing central zone of an open field. They also showed anhedonic behavior on the sucrose consumption. However, there was no difference in immobility of forced swimming test. According to our results, acid-induced hyperalgesic rats display pain-related elevation on duration of light sleep, anxious behavior, and anhedonic depressive behavior. Thus, our data suggest that the acid-induced pain is an appropriate model of fibromyalgia.
