背景及重要性
失智人口上升已是不可忽視的高齡議題,持續以在地老化為目標營造一個失智者及家庭皆能受到尊重、自主、平等的共融友善社區成為中央與地方政府共同的施政目標。而友善居民為建構高齡友善社區之要素之一,改善在地民眾對失智症的固有觀點可從提升其認知層面做起。故此,從社區關鍵人角度來了解當地社會對失智症汙名的現況,對未來規劃民眾具備在地文化內涵的知能教育是相當重要。
研究目的
從社區關鍵人角度探討在地失智症社會汙名化的現況及自覺如何消弭該現象之方法。
研究方法
採探索性研究法進行質性資料收集,透過立意取樣從台灣南部屏東縣邀請與失智者有關連的社區關鍵人,包含村里長、社區發展協會人員、警務人員、商家等,運用事先擬好的訪談指引進行半結構式訪談。之後運用內容分析法進行資料分析。
研究結果
根據訪談文本進行分析共分三大範疇:第一範疇是「從社區關鍵人角度探討產生失智症社會汙名化原因」,包括兩個主題:(1)他們是很難控制思緒的人,(2)他們是麻煩製造者;第二範疇是「失智症者被社會汙名化結果」,包括三個主題:(1)他們會被負向眼光看待,(2)他們是被疏離的一群,(3)他們常被標籤化;第三範疇則是「自覺減少社會對失智症汙名化的方法」,包括三個主題:(1)透過有效的宣導增加民眾對失智症的正確認識,(2)透過瞭解與互動提升同理心,(3)失智教育應儘早普及於各級教育。
結論
本研究透過在地社區關鍵人深入了解當地社會對失智症汙名的狀況。研究成果可進一步做為設計社區民眾失智症支持教育課程之參考,以提升民眾對失智症的正確認識,並建構失智友善社區。
The rising population of dementia is an issue of aging that cannot be ignored, and it has become a common goal of the central and local governments to create an inclusive and friendly community where both the dementia and the caregivers are respected, autonomous, and equal. Therefore, it is important to understand the local people’s perception of the stigma of dementia from the perspective of the stakeholders in the community, in order to plan the future education of people with local cultural connotation.
An exploratory research method with purposive sampling was used to collect qualitative data from a group of community stakeholders related to dementia, including village leaders, community development association personnel, police, merchants, were invited from township in Pingtung County, southern Taiwan. The semi-structured interviews using a pre-designed interview guide was conducted. Content analysis was used for data analysis.
Three categories emerged from the interview data: The first category is ” Examining the causes of social stigmatization of dementia from the perspective of community stakeholders” including two major themes: (1) They are difficult to control their moods, (2) They are troublemakers. The second category is “The consequences of social stigmatization of individuals with dementia”, including three major themes: (1) They are seen in a negative light, (2) They are alienated, (3) They are often labeled. The third category is “ways to consciously reduce social stigma of dementia” including (1) Increase the public's understanding of dementia through effective publicity, (2) Enhance empathy through understanding and interaction, and (3) Dementia education should be popularized among students as early as possible. The research results can be further used as a reference for designing community support programs to enhance the understanding of dementia and to build a dementia-friendly community.
