19世紀中葉駐臺灣海關的外國醫生醫員，形容北臺灣淡水、雞籠為氣候極差有害健康，且瀰漫「瘴癘之氣」的地方，而南臺灣打狗一帶為氣候宜人有益健康，堪稱「中國沿海氣候最佳之地」。如此差異甚大的敘述，不僅道出臺灣氣候環境特色，也透露出19世紀中葉西方醫學觀念：環境優劣左右著人體健康。然而，這群記錄下豐富臺灣氣候環境與疾病的海關醫員，有怎麼樣的背景？如何來到臺灣？在臺灣從事怎樣的工作？有著什麼樣的醫學觀？與臺灣口岸一帶外國社群、教會的互動為何？皆是本研究將處理的面向。本研究以1871年問世，由晚清中國海關醫員針對中國對外開放口岸氣候環境與健康狀況，所撰寫的刊物《中國海關醫報》(Medical Report, 1871-1908)為核心史料，輔以同時期其他文獻。本文發現：醫員來臺原因與方式包含因母國醫療人力資源飽和造成就業困難，而隨大英帝國勢力來到臺灣，此外部分醫員因醫業生涯與研究目的加上透過親友推薦，或洋行醫師轉職，而來到臺灣；醫員與教會密切合作進行醫療工作，例如北臺灣醫員成為馬偕醫療傳教的重要力量，在南臺灣醫原則與其他外國社群共同建立打狗慕德醫院；此外，醫員亦使用大量氣象數據分析環境與健康的關係以及疾病的成因，如發現氣候「熱」、「濕」或特定人為環境，使有害的「瘴氣(malaria, malarious poison)」易導致「瘴氣熱(malaria fever)」，並透過居住於高處來避免染病。本研究希望透過駐臺晚清帝國海關醫員，從兩個面向出發，深化晚清臺灣醫療史研究以及晚清海關研究。其一，透過醫員背景、職務與工作情況，深入理解其職務生活以及醫學知識疾病觀，並拓展晚清臺灣醫療在教會醫療以外的面向。其二，透過醫員了解海關醫療勤務與相關人事制度，並探討海關人員在各口岸的生活、工作以及與其他群體之互動。將海關醫員回歸其所屬機構晚清海關之下探討，找尋醫員在晚清臺灣的角色與意義，並完備清代臺灣醫療史的圖像。以期為晚清臺灣醫療發掘更多西醫來臺的管道與面貌，並提出更多從全球史探討臺灣史的可能性。

In the mid-19th century, northern Taiwan was notorious with its climate which was harmful to health, and filled with “malaria(瘴氣)” while southern Taiwan was described as “the place with the best climate” among foreigners. Such a very different narrative not only tells the characteristics of Taiwan's climate and environment, but also reveals the Western medical concept in the mid-nineteenth century: the quality of the environment influences human health. According to Medical Report of the Chinese Maritime Customs (1871-1908) , written by the Surgeons in the late Qing Dynasty on the climatic environment of China’s treaty ports and health conditions of residents living in the surrounding areas, the “heat” , “humidity”, and certain man-made environments can easily produce harmful “malaria/ malarious poison” and lead to “malaria fever(瘴氣熱)”. Applying the environmental disease theory of Western medicine in the 19th century, the Surgeons developed a discourse on understanding the relation among Taiwan’s climate, environment and health. By changing the living environment, the Surgeons tried to improve the constitution of residents and preventing diseases. They also observed the adaptation of different ethnic groups living in Taiwan to the environment. However, what was the background of these Surgeons? Why did them come to Taiwan? What kind of medical theory did they have? What was the interaction among Surgeons, foreign communities and churches? These are all aspects that this research will deal with.
　　 Through the Surgeons stationed in Taiwan, this research hopes to deepen the study of the medical history of Taiwan in the late Qing Dynasty and the study of the Chinese Maritime Customs. First, through the background, position and work situation of the Surgeons, we can deeply understand their life and medical knowledge, and expand the aspect of Taiwan medical history in the late Qing Dynasty beyond church medical care. Second, through the Surgeons, to understand the system of medical services of the Chinese Maritime Customs and to explore the life and work of customs staff at treaty ports and their interaction with other groups. Via this research, hope to explore the role and significance of the Surgeons in Taiwan in the late Qing Dynasty, and complete the image of the medical history of Taiwan in the Qing Dynasty. It hopes to discover more channels and features of Western medicine in Taiwan. And put forward more possibilities for exploring Taiwan's history from the perspective of global history.
