  • 人體主要會經由食物暴露到多氯戴奧辛(polychlorinated dibenzo-p-dioxins, PCDDs)、多氯呋喃(polychlorinated dibenzofurans, PCDFs)、擬似戴奧辛之多氯聯苯(dioxin-like polychlorinated biphenyls, DL-PCBs)化學物質而蓄積於人體內,嬰兒時期,母乳及嬰兒配方奶粉為主要食物來源。本研究有三個目的:(1)收集台灣南部地區母乳樣品測量17種PCDD/Fs及12種DL-PCBs的濃度。(2)台灣南部地區零至一歲嬰兒的飲食調查。(3)推算台灣南部地區嬰兒體內戴奧辛暴露量。
    2007年至2008年間在台灣南部地區當地醫院及月子中心募集參加者(年齡分布22~41歲,32.4 ± 3.92歲),共收集84個母奶樣本。依據美國環保署1613B和1668A的公告方法,利用高解析度氣相層析儀暨高解析度質譜儀分析樣本中17種PCDD/Fs和12種DL-PCBs同源物。分析母乳結果為17種PCDD/Fs濃度中位數為5.89 pg WHO1998-TEQDF/g lipid,12種DL-PCBs濃度中位數為4.17 pg WHO1998-TEQP/g lipid。台灣南部地區婦女的母乳中17種PCDD/Fs及12種DL-PCBs濃度自2002年到2008年呈現遞減的情況。
    嬰幼兒出生六個月連續哺餵母乳,估計17種PCDD/Fs及12種DL-PCBs的蓄積吸收量分別為18254 pg WHO1998-TEQDF及13879 pg WHO1998-TEQP。男嬰17種PCDD/Fs及12種DL-PCBs的第1個月平均每日吸收量分別為23.7 pg WHO1998-TEQDF/kg /day及18.0 pg WHO1998-TEQP/kg /day,第6個月下降為13.8 pg WHO1998-TEQDF/kg /day及10.5 pg WHO1998-TEQP/kg /day。女嬰17種PCDD/Fs及12種DL-PCBs的第1個月平均每日吸收量分別為25.4 pg WHO1998-TEQDF/kg /day及19.3 pg WHO1998-TEQP/kg /day,第6個月下降為14.9 pg WHO1998-TEQDF/kg /day及11.3 pg WHO1998-TEQP/kg /day。本研究之成果,也許可提供評估嬰兒飲食風險相關資訊。

    Food is the major source for polychlorinated dibenzo-p-dioxins, dibenzofurans and dioxin-like polychlorinated biphenyls (DL-PCBs) accumulation in human body. In infant period, breast milk and formula milk are the major food sources. The three specific aims of this project are (1) to do a survey the PCDD/Fs and DL-PCBs levels in breast milk samples collected in southern Taiwan, (2) to investigate the dietary pattern of infants in Taiwan during the first year, and (3) to assess the PCDD/Fs and DL-PCBs body burdens in infants of southern Taiwan using the data collected from aims (1)~(2).
    The breast milk samples were collected from local hospitals and three mother care centers in southern Taiwan. The samples were collected from 84 volunteers (range: 22~41 years of age, 32.4 ± 3.92 years) in years 2007 and 2008. The levels of 17 PCDD/Fs and 12 DL-PCBs in breast milk samples were measured by high-resolution gas chromatography with high-resolution mass spectrometry (HRGC/HRMS) according to the US EPA Method 1613B and 1668A. The toxic equivalent concentrations of PCDD/PCDFs and DL-PCBs for these breast milk samples were 5.89 pg WHO-TEQDF/g lipid (median) and 4.17 pg WHO1998-TEQP/g lipid (median), respectively. In southern Taiwan, the levels of 17 PCDD/Fs and 12 DL-PCBs in human milk have been declining from 2002 to 2008.
    The first 6-month cumulative intakes (CI) of PCDD/PCDFs and DL-PCBs for infants via breast milk consumption are 18254 pg WHO1998-TEQDF and 13879 pg WHO1998-TEQP, respectively. The first month, average daily intakes of PCDD/Fs and DL-PCBs for male infants via breast milk are 23.7 pg WHO1998-TEQDF/kg /day and 18.0 pg WHO1998-TEQP/kg /day, respectively; for female infants are 25.4 pg WHO1998-TEQDF/kg /day and 19.3 pg WHO1998-TEQP/kg /day, respectively. The 6th month, average daily intakes of PCDD/Fs and DL-PCBs for male infants are 13.8 pg WHO1998-TEQDF/kg /day and 10.5 pg WHO1998-TEQP/kg /day, respectively; for female infants are 14.9 pg WHO1998-TEQDF/kg /day and 11.3 pg WHO1998-TEQP/kg /day, respectively. This data could provide valuable information for risk-benefit evaluation of infant dietary.
